Meadow Rise is led by Samantha, a Practice Manager in primary care and a former carer, coordinator, and Registered Manager in social care. Her journey—from frontline care to strategic leadership—is shaped by lived experience, academic rigour, and the quiet influence of her father, whose compassion continues to guide our values.
We operate in full alignment with Care Quality Commission (CQC) standards and local commissioning priorities, including reducing health inequalities, improving emotional wellbeing, and supporting personalised care and co-production.

Meadow Rise Care Ltd is proudly rooted in Nuneaton—a town I know not just professionally, but personally. Having lived here and worked across Warwickshire in both social care and primary care, I understand the real-world needs, pressures, and strengths of our communities. I’ve supported individuals from Warwick and Coventry to North Warwickshire, and collaborated with teams across GP practices, pharmacies, and care services. I’ve seen firsthand how systems can uplift—or overlook—the people they serve.
This grounding gives Meadow Rise a unique lens: we don’t just know the regulations, we know the rhythms of local life. We understand how families navigate care, how professionals communicate across sectors, and how emotional wellbeing is shaped by continuity, trust, and cultural nuance.
As we grow strategically across the West Midlands, we’re building partnerships with Integrated Care Boards (ICBs), local authorities, and voluntary sector organisations to co-design care that’s scalable, inclusive, and emotionally intelligent.
