If someone chooses to move forward with Meadow Rise Care Ltd, we begin by carrying out a full care plan assessment and risk assessment—always guided by the principle of least restrictive but safe. This isn’t a tick-box exercise; it’s a collaborative conversation where we listen deeply to the person and those who know them best.
We explore not just what support is needed, but what outcomes and goals matter most to them. Whether it’s feeling confident walking to the garden, managing medication independently, reconnecting with a community group, or simply having a calm morning routine—we tailor care around what they want to achieve, not just what needs to be done.
We support informed choice every step of the way, making sure the person and their family understand options, feel confident in the plan, and know they’re in control. We agree together on times, durations, start dates, and how care will be introduced—always at their pace.

There’s no charge for this stage, and no pressure to commit until they feel truly comfortable. We take time to skill-match based on experience and training, but we also go deeper. We consider personality, communication style, shared interests, and emotional energy—asking ourselves, who would this person feel safe with? Who might bring out their confidence, humour, or calm? The goal is for the carer to feel like a trusted companion—more like a friend or family member than a stranger.
We know that the right relationship can transform care from a task into a moment of joy, dignity, and empowerment. That’s why carer matching is never rushed or random—it’s a thoughtful, emotionally intelligent process that puts the person at the centre.
Once care begins, we review the plan monthly for the first few months, until things feel stable, settled, and emotionally safe. After that, we move to six-monthly reviews, unless more frequent support is needed. But we know that life isn’t static—and neither is care.
As people settle in, things can change. You might find that your preferred times, days, or durations shift. You might decide you’d like more support—or less. You might realise that your companion isn’t quite the right fit. And that’s completely okay.
We remain flexible, fluid, and emotionally attuned. Just let us know, and we’ll come out to review things together. We’ll listen, reflect, and adjust the plan to make sure it still feels right for you. Whether it’s changing visit times, introducing a new companion, or revisiting your goals, we’re here to make care feel empowering, not rigid.

We’ve developed our own Resource Hub, which can be accessed digitally or brought to your home. During your care planning visit or review, we’ll sit down together and explore what outside support you might benefit from—whether that’s financial help with heating bills, mobility aids, emotional support groups, or trusted tradespeople for home repairs. We help you identify what’s needed, and then support you to access it, making referrals, sharing contact details, and even helping with forms or phone calls if needed.
We don’t just signpost—we walk alongside you. Our goal is to make sure you feel informed, empowered, and emotionally supported, with access to the right people, companies, and services to help you live well
We don’t just set up care and walk away—we stay in touch. Every person we support is offered regular wellbeing check-ins from our Registered Manager, tailored to their preference: weekly, fortnightly, or monthly. These aren’t formal tick-box calls—they’re genuine conversations to see how you’re feeling, how things are going, and whether anything needs adjusting.
We believe it’s important to know who’s responsible for your care and support—and to feel emotionally safe in that relationship. That’s why our manager doesn’t hide behind a screen. They’re present, accessible, and available 24/7 for reassurance, troubleshooting, or simply a listening ear. Whether it’s a quick chat, a home visit, or a deeper review, we’re here.
These check-ins help us stay emotionally attuned, spot changes early, and make sure your care continues to reflect your goals, preferences, and wellbeing. They also build a bond—so you know you’re not just a name on a rota, but a person we genuinely care about.
